Manifold Failure
Following are photographs of a Manifold that failed during Water Tender filling at a multi-alarm fire. An investigation
is underway to determine the exact cause of the failure or what led up to the event. Two FFs in vicinity of the
manifold were treated for minor injuries. NFPA 1962 - Standard for the Inspection, Care, and Use of Fire Hose, Couplings, and Nozzles and the Service
Testing of Hose - 2008 Edition states that appliances and couplings shall be inspected periodically. The inspection
period varies with the type of materials used in construction. A minimum of an annual inspection is required for
some appliances while a quarterly inspection is required for appliances constructed of dissimilar metals. Refer to
NFPA 1962 for details when establishing an inspection SOP for these appliances. Always refer to the manufacturers
recommendations for any and all maintenance and service specifications. Appliances such as this manifold should be regularly inspected for operation, function, and visually inspected for
cracks, dents, scratches or other physical damage that may weaken the appliance housing or valves and cause a
catastrophic failure. At the same time gaskets and valves should also be inspected and lubricated as recommended by
